Moda Health medical networks

As our healthplan administrator, Moda Health offers Zoomers and their dependents more access through the Aetna® PPO Network through Aetna Signature Administrators®.

You’ll be assigned to a network based on the coverage region where you reside:

• Oregon/Southwest Washington (Cowlitz and Clark Counties)
• Northwest Washington
• All other areas

The in-network physicians and clinicians will vary based on your coverage region.

Narrow your results and filter by specialty, telehealth providers, finding care while traveling, and finding care for your out-of-area dependent, if applicable.

Out-of-network physicians and clinicians will not display in search results.

Your in-network physicians and clinicians may vary based upon your coverage region.

• For all Zoomers – PeaceHealth Network, which includes:
• PeaceHealth facilities and PeaceHealth employed professional physicians and clinicians, including ZoomCare and PeaceHealth Joint Ventures
• Clinically Integrated Network community physicians and clinicians
• Slocum Orthopedics professional physicians and clinicians
• Willamette Valley Cancer Institute: Oncology Associates of Oregon
• Seattle Children’s Hospital facility services and professional services
• Zoomers residing in Oregon and SW Washington (Cowlitz and Clark counties) – Connexus PeaceHealth Network
• All other Zoomers and out-of-area dependents – Aetna® PPO Network
