Moda Health medical networks

While you will have access to the Aetna® PPO Network through Aetna Signature Administrators, the medical plan is administered by Moda Health.

You’ll be assigned to a home network based on where you live (Oregon/SW Washington (Cowlitz and Clark Counties), NW Washington, or remote). The providers in Tier I will be the same across all three plans, but the Tier II providers will vary based on where you live.

*Although Family Care Network will display as in-network in the Aetna network provider directory, services provided by Family Care Network are not covered under the PeaceHealth medical plans.

• PeaceHealth facilities and PeaceHealth employed professional providers, including ZoomCare and PeaceHealth Joint Ventures
• Clinically Integrated Network community providers
• Slocum Orthopedics professional providers (the surgery center is Tier II)
• Willamette Valley Cancer Institute: Oncology Associates of Oregon
• Seattle Children’s Hospital facility services are covered under Tier I; professional services are covered under Tier II (for Zoomers residing in NW Washington)
