For existing in-network providers: Updating your in-network relationship
Whether you need to add a new provider (a credentialing step) to your in-network clinic or you’re expanding your medical group (a contracting step) and need to add a new clinic to your listing, we’ll walk you through the steps you need to take.
For providers who’d like to join the Moda Health network
If you aren’t currently credentialed or contracted with Moda Health, follow this 5-step process to join our network.
1. Nomination panel
Confirm that your specialty is listed in your area, meaning we are accepting applications at this time. See the first step on the Apply page.
2. Application
Gather your W-9 and provider information and get ready to complete the application documents.
3. Contracting
We’ll share provider policies, contracts, and fee schedules. Together we’ll negotiate and make adjustments.
4. Credentialling
We’ll verify certifications, run background checks and more, taking anywhere from 6 to 12 weeks. All you’ll need to do in this phase is answer any questions we have along the way.
5. Onboarding
Once the contract is signed, we’ll welcome you to the Moda Health network with helpful educational presentations and tools and introduce you to your Provider Relations team.
