Getting Started
Once you are registered and active, login to Medical Benefit Tracker. Remember that User IDs and Passwords are case sensitive. Every 90 days you will need to change your password. See security and password rules for more details.

Subscriber/Patient Lookup
There are two ways that you can look up patient information. If you have the subscriber ID, enter in Subscriber ID field without dashes. If you were not provided with an ID, enter in patient's Last Name, First Name, and Birth Date. These fields are not case sensitive but correct spelling is required. After you have filled out the appropriate fields, press the "Go" button. If the patient has more than one plan coverage with Moda Health, an error message will prompt you to call Moda Health's customer service for more information.

New Search
To view information on a different patient, click on New Search located at the top of each screen. This will take you to the New Search screen. (If you need information on a different patient in the same family, you need only to return to Eligibility.)

Eligibility
The Eligibility page lists all the family members for a subscriber and effective dates. The effective dates reflect the patient's coverage on their current plan. Terminated members show in red.

Check Eligibility For Another Date
A patient may have different benefits prior to the effective date and a different group and/or employer during their prior coverage. To check prior eligibility (back 18 months), use the "Check Eligibility For Another Date" fields. Eligibility may also be checked for dates two months into the future. Note that the family members listed could change as the eligibility date changes.

Select a Function to Move Between Pages
To view Claims or Referrals for a member, use the drop down box next to the person's name. To return to the Eligibility page from Claims or Referrals, click on Eligibility located at the top or bottom of the page.

Benefits
The benefits page lists basic benefit information about each patient including copayment and deductible information. The page also contains contact phone numbers for additional benefit information or questions. Patients may also have notes listing further details about their plan.

The benefits shown will be for the current date unless the eligibility date has been changed. This date will be displayed at the top of the benefits page.

Claims and Claims Detail
Claims and Claims Detail allows you to see details for claims associated with a patient. For security purposes, you will only see claims activity associated with your clinic or providers' tax id number. Claims are listed with the most current first. Claims Detail can be seen on all paid claims by clicking on the specific claim number. To give your patient a printed copy of the Claim Detail screen, click on the "Printable" button at the bottom of the Claim Detail page. Then click on the printer icon in your browser to print the page.

Optional Claims Search
You can narrow your list of claims displayed by using the "Optional Claims Search." You can search by date range (beginning date, end date, or both) or by CPT Code. Once you activate the search by clicking on GO only claims which meet all of the search fields selected will show.

Referral Inquiry
Referral Inquiry allows you to look up the details and status for referrals and authorizations. For security purposes, you will only see referral and authorization activity associated with your clinic or providers' tax id number. Referrals are listed with the most current date spans first.

Referral and authorization history will be available for 18 months back starting with dates of service after August 1, 2000. History prior to August 1, 2000 can be obtained by contacting Moda Health referral unit.

Optional Referral Search
You can narrow your list of referrals/authorizations displayed by using the "Optional Referral Search." Once you activate the search by clicking on GO only referrals which meet all of the search fields selected will show. Below are explanations of the search fields:

  • Status -- by selecting an option in the drop down box (authorized, in process, partially authorized, not authorized, or unknown) referrals with this status will display.

  • Referred to Specialist -- by selecting a specialist's name in the drop down box, only referrals/authorizations to this specialist will display. This function will be of most use to a Primary Care Physicians.

  • Exclude Expired Referrals -- by checking this box, all referrals/authorizations prior to today's date will not display.
