You will need to have your ID card with you when you get care from a provider. We provide several options to help you with that.

 

Download your ID card or order a new one

You can download a PDF of your ID card or order a new one to be mailed to you. Simply log in to your Member Dashboard.

 

Get the ID card app

Get our ID card app to access the card on your phone. The app is available for iOS and Android devices.
