Get started by reviewing the steps below. If you are joining a clinic that is already contracted with us, you do not need to review the following. Instead, please complete a credentialing application now.

Complete and submit our new provider network participation request form. Please note that it may take up to 60 days for us to respond.
We'll review your application and let you know if the panel is open in your area. If so, you are eligible to be a participating provider. We'll contact you to let you know how to continue with the process.
If the panel is not open in your area at this time, please do not complete the credentialing application. You may reapply a year after your request.
If the panel is open in your area, you must complete a credentialing application and submit it to us by email, fax or regular mail.
