Moda Health group plans

If you have Moda Health care coverage through your employer, you and your family may have access to medical, dental, pharmacy and vision benefits based on your employer's offerings. You'll love everything you can do at Member Dashboard - see your prescription history, check medication costs, find a pharmacy, review claims and more. This tool helps you make the most of your pharmaceutical care.

As a member, you'll enjoy a diverse provider network, friendly customer service representatives, and our Member care resources tools to help you stay healthy. You can access all of these from Member Dashboard.

Each Moda Health medical plan covers 100 percent of in-network preventive care, including women's annual exams, well-baby care, routine physicals and immunizations.

Dental benefits

Healthy teeth are happy teeth. With our individual and family dental coverage, you'll have access to Delta Dental, the nation's largest dental network. Your smile will thank you, wherever you roam. You can access all of these from Member Dashboard.

When you need a dentist, just tap into the extensive Delta Dental Network for care, near or far. Delta Dental plans have participating providers who will not charge more than your plan's allowed amount.

Pharmacy

Preferred drug program

This program uses tiers to provide the right prescriptions at the right prices. You and your provider can choose between value, select, preferred, non-preferred, preferred specialty or non-preferred specialty tier drugs. Each tier has a different copay or coinsurance amount, based on your benefit.

Pharmacy network

Based on your plan, you have access to one of two pharmacy networks, the Navitus network or the Northwest Prescription Drug Consortium. You should use your member ID card to verify which network your plan uses.

Navitus network
The Navitus Network includes access to over 95% of pharmacies in Oregon, plus more than 58,000 pharmacies nationwide. This means you can fill prescriptions almost anywhere, including these local and national drug store chains:

  • Costco
  • CVS
  • Fred Meyer
  • Rite Aid
  • Safeway and Albertsons
  • Walgreens
  • Walmart

Northwest Prescription Drug Consortium (MedImpact)
The Northwest Prescription Drug Consortium is supported by MedImpact, a Pharmacy Benefits Manager. MedImpact operates a robust pharmacy network in Oregon and across the nation that includes both independent pharmacies and national chains.

Mail-order services

If you have a chronic health condition, our mail-order services may be for you. This program is open to members who require ongoing treatment for conditions like high blood pressure, asthma or diabetes.

If your plan allows, they can order up to a 90-day supply of medication. To get started, just visit Postal Prescription Services (PPS) online or call for more information.
