Contracted providers will need to first complete an Electronic Services Agreement.
Then, they can register online to create an account.

Members have access to several networks to search for a provider based on location and needs. Please check ID cards to determine primary network.
Moda Select Network
The Moda Select Network is for residents living in the following counties: Ada, Adams, Bannock, Bingham, Boise, Canyon, Caribou, Elmore, Gem, Minidoka, Oneida, Owyhee, Payette, Power, and Washington.
National networks
Moda members have several options for when outside their service area.
