Getting quality care is easier and more affordable when you see "in-network" providers. These doctors, hospitals and clinics agree to accept your insurance at lower rates and meet quality standards. Using an in-network provider keeps your out-of-pocket costs low.
If you see an out-of-network provider, it’s important to know that your benefits only cover a percentage of the maximum plan allowance for these services. Out-of-network providers may also bill you for the difference between the maximum plan allowance and their billed charges. This is known as balance billing. In-network Moda Health providers can’t do this.
Our individual and family medical plans include a provider network that connects you with care in your hometown and across the country. As an OEBB member, your health plan includes access to this provider network.
This is one of the largest PPO networks in the state of Oregon. It includes thousands of primary care physicians and specialists working together with Moda Health to help keep you healthy.
It’s important to remember you may pay more for services from out-of-network providers than from in-network providers. Out-of-network providers may also bill you for the difference between your maximum plan allowance and their billed charges. This is known as “balance billing.” In-network providers don’t do this. When you hit the road, care is never far. Our travel network comes with each medical plan in Oregon. When traveling outside of Oregon, members can access in-network care through the First Health Network. Any dependents designated as living outside the primary network area can also use First Health Network to receive care at the in-network benefit level. So go on, explore the world.
Please note, the Moda Health travel network is not an alternative primary network. Members must seek in-network services whenever possible, and preauthorization is required for in-patient services.
Don’t see your provider in the directory?
To nominate your medical provider, please fill out our medical provider nomination request form. Once completed, return the form to your provider's office, and let them know you would like them to initiate a network participation request with Moda Health.
Please note: New provider participation is contingent on credentialing approval, network needs, state and federal regulations, and other factors.
If you are a Retiree and/or are enrolled in COBRA and live outside Oregon, Southwest Washington (Pacific, Wahkiakum, Cowlitz or Clark counties), or Idaho, you can find a provider or pharmacy near you.
On the provider search page, select the PHCS option under “Front of Card” as indicated below. You do not need to select anything on the “Back of Card” section.
