You'll choose a plan that includes either Summit or Synergy network. Your choice connects you with a medical home a group of primary care professionals working together to support your health. Your medical home will help you access care when and where you need it.
Over time you'll get to know your care team and create a road map to get and stay healthy. It all comes with lower premiums and more ways to feel better.
This unique network gives members in eastern Oregon access to quality care at the most affordable costs. Summit Network includes a team of local and regional medical, vision and pharmacy providers who work together to coordinate care. Members can see providers at nearby hospitals and clinics or at certain Portland Metro hospitals for specialized needs. Folks in Baker, Gilliam, Grant, Harney, Lake, Malheur, Morrow, Sherman, Umatilla, Union, Wallowa and Wheeler counties can pick a plan that includes this network.
For a more integrated care experience, this fully insured network brings together providers from OHSU, Salem Health, Salem Clinic, Adventist Health, Legacy Health, Tuality Healthcare, Willamette Valley Medical Center, Samaritan Health Services, Asante, and PeaceHealth to serve the Portland Metro, SW Washington, Salem and Eugene communities. Folks in Clark, Multnomah, Clackamas, Washington, Yamhill, Marion, Polk and Lane, Jackson and Josephine counties can pick a plan that includes this network.
Beginning October 1, 2015, Synergy is expanding to include Clatsop, Columbia, Hood River, Klamath, Lincoln, Tillamook and Wasco counties. Columbia Memorial Hospital, Mid-Columbia Medical Center, Samaritan Health, and Sky Lakes Medical Center will serve members in these expansion counties.
